Clear Aligners: What Actually Determines a Good Result

Aligner treatment succeeds or fails on a handful of factors, and most of them are decided before the first tray is worn.

Case selection comes first

Clear aligners are excellent for many crowding and spacing problems and for certain bite corrections. They are less predictable for large tooth movements, significant skeletal discrepancies, and complex rotations of some tooth types.

A clinician who tells you plainly that your case is better served by fixed appliances or a specialist referral is giving you useful information, not turning away work.

Wear time is not negotiable

Aligners only move teeth while they are being worn. Consistently falling short of the prescribed daily wear time causes trays to fit poorly, tracking to drift from the plan, and treatment to extend.

If your routine makes long wear difficult, discuss it before starting. Adjusting the plan is far easier than rescuing a case that has fallen behind.

Attachments and refinements are normal

Small tooth-colored attachments are often bonded to specific teeth to give the aligners something to push against. Refinement stages, where a new set of aligners is produced partway through, are a routine part of achieving detail rather than a sign that something went wrong.

  • Wear trays as prescribed, including overnight
  • Clean trays daily and store them in their case
  • Attend review appointments so tracking can be checked
  • Expect a refinement stage in many cases

Retention protects everything

Teeth drift throughout life. Without retention, some of the correction achieved will be lost. Retainers are not an optional extra at the end; they are the mechanism that preserves the result you paid for.
